Step 1: BUY
Purchase a minimum of $300 in HP color inkjet and/or LaserJet print cartridges from the menu of qualifying product numbers below. Purchases must appear on one invoice, and must be made between 9/01/03 and 12/17/03.
Step 2: SEND
Complete the online claim form, click submit, print, and mail it to HP along
with your proof of purchase. Your proof of purchase must contain the original
UPC
symbols
of
qualifying products and a copy of the invoice or packing slip with the purchase
prices circled. Your request must be postmarked 45 days from purchase date.
